Registration Information
Information that is required for registration includes:
- Two proofs of residency: one must be a deed or lease for housing and the other can be a utility bill or request for utility service with an address
- Copy of birth certificate
- Copy of social security card
- Original Mississippi immunization compliance form
- Withdrawal form with grades or last report card from previous school
- Copy of custody or guardianship papers (if applicable)
- Completed registration forms
